    The primary role of the Nurse Coordinator is to ensure coordination of patient care across the continuum. This includes communication with patients/families, staff and providers, as well as rotating resident.

    Responsibilities

    • 1. Main duties:
      • 1.1 Coordinates initial patient contact by attending Tumor Board Conference and compiles necessary list of presented patients, identifying additions or deletions in collaboration with attending physicians and house staff. Takes meeting minutes in compliance with YNHH Cancer Committee.
    • 2. CLINICAL PRACTICE
      • 2.1 Manages incoming referrals
    • 3. CLINICAL PROGRAM
      • 3.1 Assists in the coordination of the clinical program
    • 4. CLINICAL PRACTICE
      • 4.1 Manages incoming referrals
    • 5. CLINICAL PROGRAM SUPPORT
      • 5.1 Assists in the coordination of the clinical program
    • 6. CLINICAL
      • 6.1 Obtains patient histories. Alerts house staff to significant findings and arranges evaluation or follow-up when necessary.
    • 7. RESEARCH
      • 7.1 Collaborates with investigators and protocol personnel

    Qualifications

    EDUCATION


    Bachelor's Degree in Nursing: Connecticut RN license


    EXPERIENCE


    A minimum of three (3) years experience in adult inpatient services, ambulatory oncology service or operating room.


    SPECIAL SKILLS


    Knowledge of physiology and disease involving the disease unit specialty and familiarity with tumor staging. Excellent communication skills.
